By submitting the above form, I agree that I am the authorized parent, guardian or leader of the performer(s) and hereby give my permission and consent to participate in the 2010 Katy Rice Harvest Festival. I agree that neither the Katy Area Chamber of Commerce, the Rice Harvest Festival nor any of its representatives will be held liable for injury, lost items or the like. Further, I herby agree that the submission of my entry constitutes permission for the Katy Area Chamber of Commerce or the Sponsor to use the entry for editorial, advertising or promotional purposes without further compensation. I/we agree to the guidelines and conditions of the Festival. I/we declare that the information above is true and complete. I/we understand that all applications must be completed and signed before being accepted by the Katy Rice Harvest Festival Committee.
